Consumer Health Data Privacy Notice
This Notice supplements our Privacy Policy and describes how Mavi handles “consumer health data” as defined by state consumer-health-privacy laws, including Washington’s My Health My Data Act and Nevada’s SB 370.
1.About This Notice
This Notice explains how we collect, use, and share consumer health data, and the rights you have with respect to it. Where it conflicts with our general Privacy Policy for consumer health data, this Notice controls.
2.Scope
“Consumer health data” means personal information linked to you that identifies your past, present, or future physical or mental health status. This Notice applies to such data we collect through the Services.
3.Categories We Collect
We may collect consumer health data such as:
- health conditions, symptoms, and intake responses you provide;
- treatments, medications, and products you seek or purchase;
- information about your interactions with providers; and
- data that could reasonably indicate a health condition, such as certain purchases.
4.Sources
We collect consumer health data from:
- you, directly, when you complete an intake, place an order, or contact us;
- the licensed providers and pharmacies who serve you; and
- your devices, through cookies and similar technologies you have permitted.
5.How We Use It
We use consumer health data to route your intake to a provider, facilitate treatment and fulfillment, operate and secure the Services, communicate with you, and comply with legal and pharmacy obligations. We do not use it for targeted advertising.
6.How We Share It
We share consumer health data only with:
- the licensed providers and pharmacies involved in your care;
- service providers (“processors”) acting on our behalf under contract; and
- authorities when required by law.
We do not sell consumer health data. We will not sell it without your valid, written authorization as required by applicable law.
7.Your Rights
Subject to applicable law, you have the right to:
- confirm whether we are processing your consumer health data and access it;
- have your consumer health data deleted, including by our processors;
- withdraw consent to collection and sharing; and
- appeal a denial of a request.
8.How to Exercise Your Rights
Submit a request through Your Privacy Choices or by emailing privacy@mavihealth.co. We will verify your identity and respond within the timeframe required by law. An authorized agent may submit a request on your behalf with proof of authorization.
9.Data Security
We apply the same safeguards described in our Privacy Policy (encryption, access controls, and monitoring) to consumer health data.
