Telehealth Informed Consent
This document explains what telehealth is, its benefits and risks, and what you are consenting to when you use Mavi to connect with a licensed healthcare provider. Please read it before starting a consultation.
1.What Is Telehealth
Telehealth is the delivery of healthcare services using secure electronic communications, rather than an in-person visit. Through Mavi, a licensed provider reviews the health information you submit and, where appropriate, provides evaluation, treatment, or a prescription.
2.How Mavi’s Telehealth Works
You complete an online medical intake. A licensed, independent provider reviews your responses, may request more information, and determines whether treatment is appropriate. If so, any prescription is sent to a licensed pharmacy for fulfillment.
3.Expected Benefits
- convenient access to licensed providers without an in-person visit;
- timely evaluation and, where appropriate, treatment; and
- secure, private communication with your care team.
4.Potential Risks & Limitations
As with any care, telehealth has limitations. Because the provider does not examine you in person:
- a condition may go undiagnosed or be misjudged if information is incomplete or inaccurate;
- not every condition can be evaluated or treated via telehealth; and
- technical issues could delay or disrupt care.
You agree to provide accurate, complete information and to follow up in person when advised.
5.No Emergency Services
Telehealth is not for emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room. Do not use the Services for urgent or emergency care.
6.The Provider-Patient Relationship
Care is provided by independent, licensed providers, not by Mavi. The provider exercises independent professional judgment. A prescription is issued only if the provider determines it is medically appropriate, and is never guaranteed.
7.Prescriptions & Compounded Medications
If a provider prescribes treatment, it is dispensed by a licensed pharmacy. Some products are compounded and may not be FDA-approved. You should review all instructions and discuss any questions or side effects with your provider or pharmacist.
8.Your Responsibilities
- provide truthful, complete medical information;
- use any medication only as directed and only for yourself;
- report side effects or concerns promptly; and
- seek in-person or emergency care when appropriate.
9.Privacy of Your Health Information
Your health information is handled as described in our Privacy Policy and Consumer Health Data Privacy Notice. Communications are transmitted securely.
10.Voluntary Consent & Right to Withdraw
Your participation in telehealth is voluntary. By continuing, you consent to receive care via telehealth as described here. You may withdraw your consent at any time, though it will not affect care already provided.
